The Reversal of Policy Environment and Operation Mechanism of Palliative Care Service

摘要: 本文从揭示我国缓和医疗服务遭遇的发展政策环境良好与实际运行受阻的"二律背反"现象入手, 介绍当前缓和医疗服务法律法规及制度建设情况, 分析我国医疗卫生体系底层运行机制客观阻碍缓和医疗服务发展的成因、路径和行为, 并指出这一机制下不同性质的医疗机构面对缓和医疗服务的不同行为方式。相信随着我国医疗卫生支付方式的改革, 原有机制将会发生相应变化, 并在多方面促进缓和医疗服务发展。Abstract: This paper, from the "dichotomy" phenomenon of good policy environment and obstruction of actual operation in the development of palliative care service in China, introduces the current laws and regulations and system construction of palliative care service, and analyzes the causes, paths and behaviors of the underlying operation mechanism of China's medical and health care system that objectively hinders the development of palliative care service. It also presents how medical institutions of different natures respond differently to palliative care service. We believe that with the reform of China's health care payment method, the original mechanism will change accordingly and promote the development of palliative care service in many ways.
