  • 摘要: 临床思维是临床医学人才胜任力培养的核心要素,要求医务人员在信息不完整、证据不确定及诊疗方案多元的临床情景下,综合运用专业知识、临床经验并兼顾患者个体差异,作出合理的判断与诊疗决策。传统笔试、病例分析及客观结构化临床考试(objective structured clinical examination, OSCE)等评价方式虽可有效测评学习者的医学理论知识与基础临床能力,但对复杂临床情境下动态推理过程的捕捉能力有限。脚本一致性测试(script concordance test,SCT)为解决上述问题提供了新思路。该工具以脚本理论为基础,通过设置具有不确定性的典型临床情境,考察学习者对新增信息的研判能力,分析其对诊断假设、检查选择及治疗决策的调整思路,并以专家组意见为参照进行加权评分,以判断学习者临床思维与专家模式的一致性。SCT可还原真实临床决策逻辑,适用于不同专业与培训阶段,在形成性评价中优势突出,有助于及时发现学员在信息分析、假设修正及利弊权衡等方面的思维模式及潜在缺陷。目前,SCT在我国临床医学教育中的应用仍处于初步探索阶段。后续可结合国内医学教育实际,开发SCT标准化题库与反馈机制,引入人工智能技术辅助教学反馈,以提升临床思维培养效能与医学教育质量。

     

    Abstract: Clinical reasoning is a core competency in the cultivation of competent medical professionals, requiring practitioners to integrate professional knowledge, clinical experience, and individual patient variability to make sound judgments and therapeutic decisions under conditions of incomplete information, uncertain evidence, and multiple treatment options. Traditional assessment modalities-including written examinations, case analyses, and the objective structured clinical examination (OSCE) -can effectively evaluate learners' theoretical knowledge and fundamental clinical skills, yet they have limited capacity to capture the dynamic reasoning processes deployed in complex clinical scenarios. The script concordance test (SCT) offers a novel approach to address this gap. Grounded in script theory, the SCT presents typical clinical situations with inherent uncertainty, examining learners' abilities to interpret new information and their adjustments in diagnostic hypotheses, test selection, and treatment decisions. Responses are scored by comparison with a reference panel of experts using a weighted scoring system, thereby assessing the degree of concordance between the learner's clinical reasoning and expert patterns. The SCT authentically mirrors realworld clinical decisionmaking logic, is applicable across various specialties and training stages, and demonstrates particular strength in formative assessment by facilitating early identification of learners' thinking patterns and potential deficiencies in information analysis, hypothesis revision, and riskbenefit balancing. At present, the application of SCT in Chinese clinical medical education remains in its preliminary stages. Future efforts should focus on developing standardized SCT item banks and feedback mechanisms tailored to the domestic educational context, and incorporating artificial intelligence-assisted technologies to enhance instructional feedback, thereby improving the effectiveness of clinical reasoning training and the overall quality of medical education.
