“3M护心育能计划”基线调查:北京协和医院急诊科住院医师心理健康状况与压力源的混合方法研究

“3M Mental Care Empowerment Program” Baseline Survey: A Mixed-Methods Study on Mental Health Status and Stressors of Resident Physicians in the Emergency Department of Peking Union Medical College Hospital

  • 摘要: 目的 调查急诊科住院医师的心理健康状况及其压力源,旨在为“3M护心育能计划”提供基线数据支撑。方法 采用解释性序列混合方法设计,于2025年9月——2026年2月对北京协和医院急诊科住院医师进行问卷调查。基于6个标准化量表评估住院医师的心理健康状况,统计抑郁、焦虑、睡眠质量问题及职业倦怠阳性检出率,并评估一般自我效能感与心理弹性水平。采用Spearman秩相关法分析各量表评分之间的相关性;进一步结合半结构化访谈,对量化结果进行深化与解释。结果 共纳入44名急诊科住院医师(其中40人接受访谈),其抑郁症状阳性检出率为34.1%(15/44),焦虑症状阳性检出率为15.9%(7/44),睡眠质量问题检出率为59.1%(26/44) ,高职业倦怠检出率为13.6%(6/44),一般自我效能感量表评分为(28.3±5.0)分(满分40分),心理弹性量表评分为(63.3± 12.7)分(满分100分)。Spearman秩相关法分析显示,一般自我效能感与抑郁(r=-0.533,P<0.001)、焦虑(r=-0.453,P=0.002)、情绪衰竭(r=-0.419,P=0.005)均呈中度负相关,与睡眠质量(该量表为反向计分)呈中度正相关(r=-0.402,P=0.007)。质性访谈提炼出压力源的五大核心主题:临床胜任焦虑、科研与毕业压力、医患沟通与人际压力、夜班与生理节律紊乱、社会支持网络的保护作用。结论 北京协和医院急诊住院医师面临较为突出的心理健康问题,一般自我效能感和心理弹性、社会支持网络是其心理健康状况与压力源的重要保护性关联因素。本研究为“3M护心育能计划”的精准干预提供了基线数据与循证支持。

     

    Abstract: Objective To investigate the mental health status and identify stressors among resident physicians in the Emergency Department (ED), and to provide baseline data for the “3M Mental Care Empowerment Program”. Methods An explanatory sequential mixed-methods design was adopted. A questionnaire survey was conducted among ED residents at Peking Union Medical College Hospital from September 2025 to February 2026 using six standardized scales to assess mental health outcomes. Positive screening rates for depressive symptoms, anxiety symptoms, sleep disturbances, and high burnout were calculated, and levels of general self-efficacy and psychological resilience were evaluated. Correlations among scale scores were analyzed using Spearman’s rank correlation coefficient. Semi-structured interviews were subsequently performed to elaborate on and interpret the quantitative findings. Results A total of 44 ED residents were enrolled, of whom 40 participated in the interviews. The positive screening rates were 34.1% (15/44) for depressive symptoms, 15.9% (7/44) for anxiety symptoms, 59.1% (26/44) for sleep disturbances, and 13.6% (6/44) for high burnout. The mean score on the General Self-Efficacy Scale was 28.3±5.0 (out of 40), and the mean score on the Connor-Davidson Resilience Scale was 63.3±12.7 (out of 100). Spearman correlation analysis revealed that general self-efficacy was moderately and negatively correlated with depressive symptoms (r=-0.533, P<0.001), anxiety symptoms (r=-0.453, P=0.002), and emotional exhaustion (r=-0.419, P=0.005), and was moderately correlated with sleep quality (reverse-scored scale) in the positive direction (r=-0.402, P=0.007). Qualitative interviews identified five core thematic stressors: clinical competency-related anxiety, research and graduation pressures, doctor-patient communication and interpersonal challenges, night shifts and circadian rhythm disruption, and the protective role of social support networks. Conclusions ED residents at Peking Union Medical College Hospital face considerable mental health challenges. General self-efficacy, psychological resilience, and social support networks serve as important protective correlates against psychological distress and stressors. This study provides baseline data and evidence-based support for the targeted implementation of the “3M Mental Care Empowerment Program”
