Abstract: Patients with gouty arthritis may encounter a range of perioperative challenges in orthopedic settings, including acute gout flares and infectious complications, which not only compromise patient well-being but may also adversely affect surgical outcomes and escalate healthcare costs. To date, there is a paucity of guidelines or consensus statements specifically addressing the perioperative management of gouty arthritis, both domestically and internationally, resulting in a lack of standardized clinical protocols. Consequently, there is an urgent need to develop evidence-based expert consensus to optimize diagnostic and therapeutic strategies. In response, the Department of Orthopedics at Peking Union Medical College Hospital convened a multidisciplinary panel of experts to formulate the Chinese Expert Consensus on Perioperative Management of Gouty Arthritis (2025), drawing upon the latest advances in the relevant literature. This consensus yields 10 recommendations addressing the potential perioperative issues and corresponding prophylactic and therapeutic approaches for patients with gouty arthritis. The recommendations encompass four key domains: preoperative evaluation and risk stratification, intraoperative technical optimization, perioperative pharmacological management, and postoperative complication management. The consensus is intended to provide guidance for the perioperative care of gouty arthritis patients, with the ultimate goals of improving surgical outcomes, reducing the risk of perioperative complications, and enhancing patient prognosis.
