  • 摘要: 2025年美国胃肠病学会(American College of Gastroenterology,ACG)发布的《成人克罗恩病诊疗指南》在制订方法、诊断逻辑、治疗目标及策略上进行了重要更新。新版指南强调症状需经内镜或影像等客观证据验证后方可归因于疾病活动,治疗目标从追求黏膜愈合拓展为临床缓解、内镜缓解及生活质量提升并重,在治疗策略上明确倡导新诊断的中重度患者及早启用生物制剂,推行"降阶梯治疗"。同时针对狭窄性病变方面,突破既往狭窄不可逆的观念,提出炎症为主型的狭窄可通过强化药物治疗改善。对特定局限病变患者,早期回盲部切除术被列为抗肿瘤坏死因子治疗之外的合理一线选择。新版指南为克罗恩病的临床诊疗提供了更具循证依据和前瞻性的管理框架,对推动个体化精准治疗具有重要的实践指导意义。

     

    Abstract: The newly released 2025 American College of Gastroenterology (ACG) Clinical Guideline for the Management of Adults with Crohn's Disease introduces important updates in methodology, diagnostic logic, therapeutic goals, and treatment strategies. The updated guideline emphasizes that symptoms should be attributed to disease activity only after verification by objective evidence such as endoscopy or imaging. Therapeutic goals have been expanded from solely pursuing mucosal healing to equally emphasizing clinical remission, endoscopic remission, and improvement in quality of life. In terms of treatment strategy, the guideline clearly advocates for early initiation of biologic agents in newly diagnosed patients with moderate-to-severe disease, promoting a top-down approach. Regarding stricturing disease, breaking the previous concept that strictures are irreversible, the guideline proposes that inflammationpredominant strictures can be improved with intensified medical therapy. For selected patients with localized disease, early ileocecal resection is listed as a reasonable first-line alternative to anti-TNF therapy. The new guideline provides a more evidence-based and forward-looking management framework for the clinical diagnosis and treatment of Crohn's disease, offering important practical guidance for advancing individualized precision medicine.
