儿科模拟教学师资核心能力要素的系统综述与主题综合分析

A Systematic Review and Thematic Synthesis of Core Competency Elements for Pediatric Simulation Educators

  • 摘要: 目的 系统整合现有文献,明确有效开展儿科模拟教学所必需的师资核心能力要素,为师资培养与评估提供循证依据。方法 采用系统综述方法,检索PubMed、Embase、Web of Science、中国知网、万方数据知识服务平台,检索时限为建库至2026年1月31日。由2名研究者根据文献纳入与排除标准独立进行文献筛选、数据提取,采用混合方法评价工具(mixed methods appraisal tool,MMAT)进行方法学质量评价,采用主题综合法对纳入文献进行编码、归纳与整合。结果 共纳入14项研究,发表时间为2019-2026年,其中中国7项、美国3项、土耳其1项、日本1项、印度1项、加拿大1项。研究设计包括3项随机对照试验、4项准实验研究、1项质性研究、2项横断面调查研究、3项非随机对照试验、1项混合方法研究。5项研究MMAT评分为100%,9项为80%,总体方法学质量为中等偏上。共提取初始编码87个,经比对合并为25个次级编码簇、8个描述性主题,最终抽象为4个分析性主题(即四大核心能力领域),包括课程设计与开发能力、教学引导与反馈能力、专业技术与情境管理能力,以及评估、反思与持续发展能力。结论 本研究提炼的四大核心能力框架,涵盖课前筹备、课堂实施、课后评估与改进的完整教学链条,可为儿科模拟师资的选拔、培训、考核及职业发展提供参考依据。

     

    Abstract: Objective To systematically synthesize existing literature, identify the core competency ele- ments essential for instructors to effectively deliver pediatric simulation-based teaching, and provide evidence- based references for faculty development and evaluation. Methods A systematic review was conducted by searching PubMed, Embase, Web of Science, China National Knowledge Infrastructure (CNKI), and Wanfang Data Knowledge Service Platform from database inception to January 31, 2026. Two researchers independently performed literature screening and data extraction according to predefined inclusion and exclusion criteria. Methodological quality was assessed using the mixed methods appraisal tool (MMAT). Thematic synthesis was employed to code, summarize, and integrate the included literature. Results A total of 14 studies published between 2019 and 2026 were included, comprising 7 from China, 3 from the United States, and 1 each from Turkey, Japan, India, and Canada. In terms of study design, there were 3 randomized controlled trials, 4 qua-si-experimental studies, 1 qualitative study, 2 cross-sectional surveys, 3 non-randomized controlled trials, and 1 mixed-methods study. Five studies received an MMAT score of 100%, while 9 scored 80%, indicating an overall moderate-to-high methodological quality. A total of 87 initial codes were extracted, which were compared and consolidated into 25 subclusters and 8 descriptive themes, ultimately abstracted into 4 analytical themes (i. e., four core competency domains): curriculum design and development; instructional facilitation and feedback; technical proficiency and scenario management; and assessment, reflection, and continuous professional development. Conclusions The four-core-competency framework identified in this study encompasses the complete teaching chain from pre-session preparation and in-session implementation to post-session evaluation and improvement, and may serve as a reference for the selection, training, assessment, and career development of pediatric simulation educators.
