心脏磁共振成像在系统性红斑狼疮相关心脏假性室壁瘤诊疗中的应用:病例报告及文献复习

Exploring the Application of Cardiac MRI in Diagnosis and Treatment of Cardiac Pseudoventricular Aneurysm Caused by Systemic Lupus Erythematosus: A Case Report

  • 摘要: 系统性红斑狼疮(systemic lupus erythematosus,SLE)是一种累及多脏器多系统的自身免疫性疾病,可导致多种心脏病变,其中假性室壁瘤为罕见并发症。本文报道1例罕见的SLE并发左心室假性室壁瘤病例(此为文献所见第7例),介绍其诊疗过程,回顾既往影像学资料;同时进行系统文献回顾,总结SLE并发假性室壁瘤的相关病例报道。该病例提示冠状动脉微循环障碍导致心肌梗死,可能是患者并发假性室壁瘤的原因,MRI心肌灌注延迟增强扫描有助于早期发现心脏病变,对此类患者的早期干预具有重要价值。

     

    Abstract: Systemic lupus erythematosus (SLE) is an autoimmune disease involving multiple organs and systems, which can lead to various cardiac pathologies, among which pseudoaneurysm is a rare complication. This article reports a rare case of SLE complicated by left ventricular pseudoaneurysm (the seventh case reported in the literature), detailing the diagnosis and treatment process, and reviewing previous imaging data. Additionally, a systematic literature review was conducted to summarize reported cases of SLE complicated by pseudoaneurysm. This case suggests that myocardial infarction caused by coronary microvascular dysfunction may be the underlying mechanism for the development of pseudoaneurysm in this patient. Late gadolinium enhancement on cardiac MRI is valuable for the early detection of cardiac involvement and plays an important role in early intervention for such patients.
